Getting Started Downloads
Forms that require a signature before payrolls may be processed.
- Form 8655 - Reporting Agent Authorization Form (Signature Required)
This Internal Revenue Service form authorizes BeyondPay to: - act as your reporting agent
- sign and file Federal employment tax returns
- make Federal tax deposits and other Federal tax payments on your behalf
- Company Authorization Agreement For ACH Use (Signature Required) This form is an agreement that outlines your responsibilities and rights in using the Automated Clearing House (ACH) bank transfer system. This is a legal contract and you should read it carefully, making sure you understand it.
- Power of Attorney (Signature Required)
This form authorizes BeyondPay to collect and remit tax funds, and file returns as an agent of your organization.
Forms and Files for Employee Census Information
- Employee Direct Deposit Authorization Agreement
Use this form if any of your employees want direct deposit. Please include a voided check from the employee's account to ensure proper crediting. For savings accounts or credit unions, please provide accurate bank name, routing/transit and account numbers.
Please note: this form is required for each employee using direct deposit.
- Form W-4 – Employee's Withholding Allowance Certificate
This form enables your employees to declare appropriate withholdings. While we do not need a copy of the actual Form W-4, we do need the information it contains (employee name, address, Social Security number, withholding status [married or single] and the number of allowances being claimed.
- Form I-9
All employees (citizens and noncitizens) hired after November 6, 1986, and working in the United States must complete Form I-9.
- Microsoft Excel - Employee Census Spreadsheet (Recommended)
This spreadsheet must be filled out for any employers with 10+ employees, and is recommended for all employers of any size. Each column marked as "Req" are required for every employee. Please fill this out carefully and completely. Failure to use a spreadsheet for import may result in an additional setup charge.

- 401(k) Agreement (Optional)
Sign this form if you wish BeyondPay to process your 401(k) plan payments on your behalf.
