• Getting Started Cover Sheet
  Use this transmittal cover sheet when faxing or sending signed forms to BeyondPay. It includes a quick checklist to ensure you provide the information we need to begin processing your payroll.

• Quick Start Payroll Submittal Form
  Use this form to report your company information to us if you have not already submitted the on-line Getting Started form.

• Form 8655 – Reporting Agent Authorization Form (Signature Required)
  This Internal Revenue Service form authorizes BeyondPay to:

  • act as your reporting agent
  • sign and file Federal employment tax returns
  • make Federal tax deposits and other Federal tax payments on your behalf

• Company Authorization Agreement For ACH Use (Signature Required)
  This form is an agreement that outlines your responsibilities and rights in using the Automated Clearing House (ACH) bank transfer system. This is a legal contract and you should read it carefully, making sure you understand it.

• Tax Filing Agreement (Signature Required)
  This form authorizes us to access your payroll bank account for payroll funds and service fees.

• Employee Direct Deposit Authorization Agreement (Optional)
  Use this form if any of your employees want direct deposit. Please include a voided check from the employee's account to ensure proper crediting. For savings accounts or credit unions, please provide accurate bank name, routing/transit and account numbers.

• Form W-4 – Employee's Withholding Allowance Certificate (Optional)
  This form enables your employees to declare appropriate withholdings. While we do not need a copy of the actual Form W-4, we do need the information it contains (employee name, address, Social Security number, withholding status [married or single] and the number of allowances being claimed.

• Laser Signature and Logo Set Up Form (Optional)
  Download this form to include a laser signature or company logo on your checks.
